If you know the lore of the mod, Inigo is a recovering Skooma addict. Falling into a crippling addiction to it after the death of his brother, leading to his EXTREMELY questionable mental state prior to meeting the Dragonborn, leading to his fantastical stories like how he met Mr. Dragonfly, or his attempted suicide, it's to the point where we don't know for certain if his story of how he attempted to kill the player on a botched mercenary job prior to the Helgen intro is even true, as Langley Longseer even flat out questions Inigo if the player is truly the person he re.
But he is EXTREMELY proud of himself for kicking his Skooma addiction. "My body misses it from time to time, but my mind is made up.", a direct quote. The mental fortitude it takes to resist an addiction as crippling as his was is insane.
So seeing him relapse like this is...really depressing in a way.
